Graphify Tooling — Code Graph Rebuild

tools/graph_watch/rebuild.py (first-party AST extraction via the graphifyy package) feeding graphify-out/, called from nightly_builder.py. Everything under graphify-out/ is gitignored build output (KAN-1137): regenerated locally, never tracked, so name it as a glob or as the route that serves it rather than as a repo file. Phase 0 and from the daemon's watch-status/rebuild endpoints.
